Sarah O’Callaghan

My name is Sarah, and I am one of the 2 care coordinators currently working at the practice. My current role is to look after our patients with long-term health conditions as well as anybody over the age of 40. You will see me or my colleague Alana once a year, where we will take your height, weight, BP, pulse and blood samples. You may then also be seen by either a nurse or GP to review the results and review any medications you are taking. If you have little to no long-term health conditions but are over 40 you may only need a review with a pharmacist which is usually done by telephone. We will inform you at your appointment what steps you will need to take next.
